What do I need to bring to my appointment?
MMTC providers request that you bring supporting documentation with your diagnosis or prior treatment to the clinic at the time of your initial appointment. In addition, please bring identification or proof of Florida residency.
Do I need the Registry ID card first?
No. In fact, a medical cannabis provider such as those at our clinics must qualify you and complete an assessment before you can be assigned a patient ID number, which you need in order to apply for the card through the state Office of Medical Marijuana Use.
Do you take my insurance?
Unfortunately medical cannabis is still federally illegal, therefore no insurance company has elected to cover cannabis-related treatments.
Is my medical information secure?
We take all necessary measures to be 100% HIPAA compliant. Any personal health information you provide is completely safe and secure.

Carl Hardy MD

Dr. Carl Hardy is a graduate of The Ohio State University College of Pharmacy and practiced as a clinical pharmacist with an interest in improving cancer chemotherapy and pain management where he also learned of the beneficial medicinal properties of Cannabis and the Cannabinoids. He later attended Ohio State University College of Medicine and completed his post Doctorate Residency at The University of Florida, Gainesville with additional qualifications in Critical Care Medicine and Pain Management. Dr. Hardy is a Board-Certified Anesthesiologist. Dr. Hardy pioneered modern anesthesia services at Baptist Medical Center-Beaches as its first Chief of Anesthesia. Throughout his career while working in Florida hospitals and surgery centers, Dr. Hardy has taken on numerous additional roles for the benefit of developing care guidelines as an anesthesiologist and health care leader including serving on Medical Executive Committees and as a Member of the Hospital Board of Trustees. I was in a car accident and as a result, have bad lower back pain with sciatica. I visited the chiropractor 2-3 times a week but I was still in constant pain with frequent sciatic flare ups. I moved to Jacksonville, FL to get new start in life and the fact that the Mayo Clinic was close (I thought would be good for me), was a deciding factor to move.

I started going to the clinic in Jacksonville Beach and the staff there is so very helpful and really do want to help everyone that comes into their office. I wanted to get off prescription medication, as I have Celiac Disease and sleeping problems since I lost my spouse.

I have been using medical marijuana to combat PTSD and chronic pain. I was using prescription medicine in pill form, 4 times a day to manage my pain and 3 pills at night to manage my sleeping problems. But since taking medical marijuana, I am almost down to not taking any pills at all. This had been a great help to also manage the stress level and anxiety issues as well.

I have been sleeping better and feel more rested in the morning. Since starting medical marijuana, I am able to concentrate/focus more at work.

If you can qualify for medical marijuana, I think you should go the nearest MMTC and seek help and get information on what types of the medical marijuana is for you, as there are so many different forms and strands to help. I still haven’t found a strand that helps me eat more, but I’m still working on that and maybe in the next few months I will be able to eat more.

Medical marijuana had truly been a blessing for me, plus it is healthier on my organs and my blood work is better since I stopped taking the medicines for my pain and sleeping issues.

A really good plus is that the staff at the dispensaries are very helpful to get you the right medical marijuana for what you need.

Using the medical marijuana has made me less dependent on my pain meds. I also have some serious issues with my digestive tract so pain meds have a tendency to upset a delicate balance that is incredibly hard to maintain. It really has made my day-to-day living so much better!
